Physiotherapy Specialist Referrals

Referrals are welcomed from any registered Health Practitioner for diagnosis or treatment planning consultations.  

To access this service, please complete a referral form (below) including relevant clinical details.  Send your referral to the email address included on the form. The referral will be processed (takes approx 7-10 days) and patient will be contacted to arrange an appointment. We also accept EDI and ERMS referrals. See our contact page for EDI details. 

Initial consultations will take one hour. Physiotherapists are welcome and are encouraged to attend the appointment with the patient in person, or via zoom where possible.  A full report detailing clinical findings, diagnosis and recommended management will be provided to the referrer within 7 days.

If there are no current ACC claims, the full cost of treatment will be charged to the patient. If there is third-party or insurer funding, we require written prior approval of funding for specialist physiotherapy services.
